Elder Care at Home Self-Help for the Elderly's Elder Care at Home program is designed to provide short-term, non-medical services to meet the sudden needs of seniors and disabled adults. Services are available to those who have recently been discharged from the hospital and require immediate assistance, need temporary care while waiting for authorization from the Department of Human Services for In-Home Supportive Services or need one-time house cleaning. Elder Care workers provide services on an emergency and temporary basis until more permanent services can be arranged. Referrals to health and social services agencies are made as appropriate.

- Personal care assistance such as helping with bathing, dressing, eating, and transferring
- Homemaker services such as meal preparation, shopping, laundry, and light housework
- Chores like heavy housework and cleaning
- A visit by a social worker to arrange other services
Eligibility
San Francisco resident aged 60 or older or an adult with a disability who is:
- Unable to safely return home from the hospital without temporary non-medical assistance, or
- In need of temporary in-home services while awaiting authorization for IHSS from the Department of Human Services, or
- In need of one-time heavy cleaning
