Home Health Care The mission of our HomeCare program is to provide quality home health services, information and education to the community, to assist the patients and their family in optimizing their quality of life, and to advocate for the mono and bilingual patient, and their families, to access health care services. Self-Help for the Elderly's HomeCare service is a state-licensed, Medicare-certified home health agency specializing in meeting the unique home health needs of the culturally diverse population in San Francisco and Northern San Mateo County.
A multi-disciplinary team of healthcare professionals provide care in the privacy and comfort of the patient's home under the direction of a physician. Our team members are bilingual and bicultural in Chinese, eliminating any language or cultural barriers.
Self-Help HomeCare and Hospice abides by the rules and regulations of HIPAA.

- Coordination of care with the patient's physician
- Care plan designed by a team of health professionals:

- Under the care of a licensed physician, podiatrist or dentist
- In need of on-going care by professionally licensed nurses, physical, occupational or speech therapists
- Homebound
- May not qualify for hospital admission and requires clinical services for medical problems as evaluated by their physician within the past 6 months
- Have acute or unstable medical conditions that make traveling to treatment centers difficult
