Hospice The mission of our Hospice program is to provide quality hospice services, information and education to the community, to assist the patients and their family in optimizing their quality of life, and to advocate for the mono and bilingual patient, and their families, to access health care services. Self-Help for the Elderly's Hospice program provides services specially designed for patients with terminal illnesses and is the only non-profit agency specializing in meeting the unique needs of the culturally diverse population in San Francisco and Northern San Mateo County.
Our Hospice program helps patients accept their prognosis and live comfortably in the privacy of their homes and in harmony with their families. For many patients, Hospice enriches their lives and enables them to live their last days to the fullest and pass peacefully with comfort and dignity. Our health care team members are bilingual and bicultural in Chinese, eliminating any language or cultural barriers.
Self-Help HomeCare and Hospice abides by the rules and regulations of HIPAA.

Services The Hospice concept of care involves a specialized health care team:
- Medical director
- Registered nurses
- Medical social workers
- Certified home health aides
- Hospice volunteers
- Chaplain
- Bereavement support
- Physical, occupational and speech therapists
- Pharmacy services
The team addresses the patient's and family's physical, social, emotional, spiritual and financial needs.
- Patients who are faced with advanced stages of any life-threatening illness, and who, according to their physicians, have a life expectancy of six month or less
- Patients who want the comfort measures that Hospice can provide rather than aggressive treatment
- Must reside in San Francisco or Northern San Mateo
