Alzheimer's Assisted Living - Autumn Glow Located in the Hayes Valley district of San Francisco, this nine-room fifteen-bed facility was completed in Spring 2001. The facility provides 24-hour care for seniors who are age over 60 and afflicted with Alzheimer’s disease and related dementia. Autumn Glow’s California State RCFE license number is 385600141. All residents who are within HUD low income criteria can receive HUD rental assistance.

Autumn Glow has three private bedrooms and six semi-private bedrooms in a two storey high Victoria building. It also consists of a decent activities/dinning room in the basement, cozy sitting common area on each floor and a spacious roof top garden. The facility accommodates both ambulatory and non-ambulatory residents. The building is convenient with public transportation and easy to find street parking. The location is close to Japan Town, San Francisco City Hall and Opera House etc.

Over 20 years of experience Self-Help for the Elderly opened its first assisted living facility in San Francisco’s Richmond District in 1989. It currently operates two facilities: Autumn Glow in San Francisco and Kwok Yuen in San Jose.
