The Support at Home program, administered by Self-Help for the Elderly (SHE), is funded by the San Francisco Department of Aging and Adult Services (DAS) and includes a copayment from participants. This program promotes independent living, dignity, and the ability to age in place. It is designed for individuals who cannot access other home care resources due to affordability barriers, such as income, asset restrictions, or other eligibility criteria.
Eligibility Criteria
To qualify, participants must meet ALL of the following requirements:
- San Francisco resident aged 60+ or adults 18–59 with disabilities;
- Need help with at least two daily living activities;
- Meet income and asset limits (under 100% AMI and less than $40,000 in assets, excluding home and one car);
- Show financial and functional need for subsidized home care;
- Be willing to share in the cost of care and participate in program evaluations;
- Not be eligible for other subsidized home care programs (e.g., In-Home Supportive Services (IHSS), Community Living Fund (CLF), In-Home Operations (IHO), and other state waiver programs)
